Qualifications

• Active RN license in Maryland is required.
• Experience in home health, med-surg, or community nursing is preferred.
• Strong clinical assessment and critical thinking skills are essential.
• Excellent communication and patient education abilities are expected.
• Own transportation and valid driver’s license for home visits.
• Proficiency with EHR and clinical documentation is required.
• Compassionate, self-motivated and able to work independently.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications, treatments, and skilled nursing procedures in a home health setting.
• Monitor and document patient progress, vital signs, and changes in condition; coordinate care with physicians, therapists and healthcare team members.
• Educate patients and family members on disease management, medications and home safety.
• Ensure compliance with state, federal and agency regulations and documentation standards.
