comForCare in Maryland is seeking a skilled registered nurse to provide in-home care as part of our Assistive Care team. You will coordinate patient care, collaborate with care teams, and educate families to support patient well-being.

• Active RN license in the state of practice.
• Minimum of 12 months of RN experience.
• Driver's license and reliable transportation in state of residence.
• Basic computer skills (MS Word and Outlook).
• Current CPR certification.

Responsabilidades

• Coordinates and oversees all aspects of patients' home care
• Collaborates with patient care teams to develop and implement care plans
• Educates families on patient condition, needs and care
• Performs head to toe physical assessments
• Checks vital signs and manages medications
• Completes documentation timely and accurately
• Regularly communicates patient progress to the clinical manager, physician and care team
• Other duties as assigned
