Always Best Care Grafton is seeking a dependable Registered Nurse (RN) to join the team on a full-time or part-time basis. The RN will provide skilled nursing care in clients' homes, perform OASIS assessments, and coordinate with families and physicians to ensure high-quality care.

The role requires strong clinical assessment, accurate documentation, and the ability to travel to client homes. On-call weekend coverage every 3rd week is required, with competitive point-based compensation and

Formación

• Current and valid RN license.
• Home Health Care experience preferred.
• Strong clinical assessment and documentation skills.
• Experience with or knowledge of OASIS assessments.
• Dependable transportation and ability to travel to clients' homes.
• Willingness to participate in on-call weekend coverage every 3rd week.

Responsabilidades

• Provide professional nursing care and clinical oversight in clients' homes.
• Complete comprehensive nursing assessments and reassessments.
• Perform OASIS assessments and required documentation.
• Coordinate care with physicians, therapists, clients, and families.
• Administer medications and treatments as ordered.
• Maintain accurate and timely clinical documentation.
