Job summary

Always Best Care Grafton is seeking an experienced Registered Nurse (RN) to join our team on a full-time or part-time basis. You will provide professional nursing care and clinical oversight in clients' homes, perform comprehensive assessments, and coordinate care with families and physicians.

The role involves OASIS assessments, care planning, and documenting musculoskeletal and clinical findings, with on-call weekend coverage every 3rd week.

Qualifications

• Current and valid RN license
• Home Health Care experience preferred
• Strong clinical assessment and documentation skills
• Experience with or knowledge of OASIS assessments
• Dependable transportation and ability to travel to clients' homes
• Dependable, responsible, and professional work ethic
• Ability to work independently in a home-care setting
• Willingness to participate in on-call weekend coverage every 3rd week

Responsibilities

• Provide professional nursing care and clinical oversight in clients' homes
• Perform Start of Care, Reassessment, Recertification, and Discharge assessments
• Complete OASIS assessments and required documentation
• Maintain accurate clinical documentation
• Coordinate care with physicians, therapists, clients and families
• Provide education and support to clients and families
• Perform wound care and nursing procedures as required
• Administer medications and treatments as ordered
