Always Best Care Grafton is seeking an RN to provide in-home nursing care on a full-time or part-time basis. You will perform comprehensive assessments, coordinate with the care team, and ensure high-quality clinical documentation.

The role requires travel to client homes, the ability to work independently, and on-call weekend coverage on a rotating basis. Join a compassionate team dedicated to safe, dignified home health care.

Formación

• Current and valid RN license.
• Home Health Care experience preferred.
• Strong clinical assessment and documentation skills.
• Experience with OASIS assessments is a plus.
• Ability to travel to clients' homes and provide in-home nursing care.

Responsabilidades

• Provide skilled nursing care per each client’s care plan.
• Perform comprehensive nursing assessments and reassessments.
• Conduct OASIS assessments and required documentation.
• Complete Start of Care, Resumption of Care, Recertification and Discharge assessments.
• Monitor health status and update care plans as needed.
• Coordinate with physicians, therapists, clients, and families.
• Deliver client and family education and support.
• Administer medications and wound care as ordered.

Full-Time or Part-Time Registered Nurse (RN)

Always Best Care Grafton is looking for a dependable, compassionate, and experienced Registered Nurse (RN) to join our team on a full-time or part-time basis.

What You’ll Do

As an RN with Always Best Care Grafton, you will provide professional nursing care and clinical oversight to clients in their homes.

Your responsibilities may include:

• Providing skilled nursing care based on each client's individualized care plan

• Completing comprehensive nursing assessments and reassessments

• Performing OASIS assessments and required documentation

• Completing Start of Care, Resumption of Care, Recertification, and Discharge assessments

• Monitoring clients' conditions and identifying changes in their health status

• Developing, updating, and evaluating individualized plans of care

• Coordinating care with physicians, therapists, caregivers, clients, and families

• Providing education and support to clients and their families

• Performing wound care and other nursing procedures within your scope of practice

• Administering medications and treatments as ordered

• Maintaining accurate and timely clinical documentation

• Helping clients maintain their comfort, dignity, safety, and independence

• Following applicable nursing standards, Medicare requirements, and agency policies

What We Offer

Competitive Point-Based Compensation

• $45 per point

• $50 per day for on-call rotations

• $0.43 per mile for eligible mileage. Mileage is not paid for travel to the first visit or from the last visit of your workday

Point Structure

• OASIS Start of Care: 2.5 points per visit

• OASIS Resumption of Care: 2 points per visit

• OASIS Recertification: 1.56 points per visit

• OASIS Discharge: 1.56 points per visit

• OASIS Non-Billable Discharge: 0.45 points per assessment

• Non-Medical Client Assessment: 1.56 points per visit

• Regular Skilled Patient Visit: 1 point per visit

What We’re Looking For

Must-Haves

• Current and valid Registered Nurse (RN) license

• Home Health Care experience preferred

• Strong clinical assessment and documentation skills

• Experience with or knowledge of OASIS assessments

• Dependable transportation and ability to travel to clients' homes

• Dependable, responsible, and professional work ethic

• Ability to work independently in a home-care setting

• Willingness to participate in on-call weekend coverage every 3rd week

Preferred Experience

• Previous Home Health Care (HHC) experience

• OASIS Start of Care experience

• OASIS Recertification experience

• OASIS Resumption of Care experience

• OASIS Discharge experience

• Experience with Medicare-certified home health

• Wound care experience

• Experience caring for elderly or medically complex clients

• Experience coordinating care with physicians and other healthcare professionals

Additional Certifications That Add Value

The following certifications or specialized training are a plus:

• Current BLS/CPR certification

• Wound Care Certification

• OASIS/Home Health training

• IV Therapy/Infusion Certification

• Dementia/Alzheimer's Care Training

• Other relevant nursing certifications or specialized training
