Job summary

Nightingale Home Healthcare is expanding and seeks full-time Registered Nurse Case Managers to deliver skilled nursing care in patients' homes. You will administer care per physician orders, teach family members, and coordinate with LPNs and aides under the Director of Clinical Services.

Qualifications

• Licensed Registered Nurse in MN.
• 1 year medical/surgical, home health, or geriatric nursing experience preferred.
• Wellsky (Kinnser) experience preferred.
• Current TB results within last 12 months.
• Current CPR Certification.
• Valid driver’s license and proof of liability insurance on automobile.
• Ability to attend 3-day orientation at our Minnetonka office.
• Ability to work 8 AM – 5 PM Monday – Friday with one weekend a month on call.

Responsibilities

• Obtain medical history from patient and family.
• Conduct brief physical examination and assess vital signs.
• Develop and implement nursing care plans.
• Provide skilled nursing care per care plan and physician orders.
• Coordinate care with LPN, Home Health Aide, and Personal Care Attendant.
• Document visits and update plans of care as needed.
• Maintain HIPAA confidentiality and participate in team conferences.
