Job summary

Idaho Home Health, part of LHC Group and a member of the Optum family, seeks an RN to provide and direct home nursing care in Meridian, ID. You will coordinate a total plan of care with physicians, agency staff, patients and families, ensuring compliance with laws and policies.

Responsibilities include high-quality clinical nursing, care coordination from admission to discharge, and accurate assessments for payer needs. A patient-focused, compassionate approach is essential.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license and access to reliable transportation.
• Preferred: CPR Certification or ability to obtain within 90 days of hire.
• Preferred: 1+ year RN experience.

Responsibilities

• Provide high-quality clinical nursing services within scope of practice and infection control standards.
• Coordinate care with the patient/client's care team from admission to discharge.
• Complete nursing assessments per program requirements and payer needs.
• Ensure eligibility and medical necessity for services as defined by payer and agency policy.
• Develop and revise individualized plans of care with other providers.
• Ensure plan of care frequency and duration meets patient needs and is updated with physician approval.
