Job summary

Gateway Regional Home Health, part of the Optum family, is hiring a Registered Nurse to provide in-home nursing care and coordinate the plan of care with physicians and the care team. The role includes supervision of staff, comprehensive assessments, and documentation to meet legal requirements.

The position offers competitive hourly pay of $36–$45 with per-visit point compensation and a comprehensive benefits package.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current CPR Certification or ability to complete within 90 days of hire.
• 1+ years of Home Health experience (preferred).
• Ability to work independently and manage time effectively.
• Strong communication, writing, and organizational skills.

Responsibilities

• Provides and directs nursing care to patients in their homes as prescribed by the physician and in compliance with laws and agency policies.
• Coordinates total plan of care with other health care professionals and exchanges information with physician, patient, family, and resources.
• Supervises home health aides and licensed nurses, provides instruction, and assigns tasks per regulations.
• Performs comprehensive assessments (OASIS), medication reconciliation, and documents visits per policy.
