Job summary

UnitedHealth Group's HomeCare Services of Haywood is seeking a Registered Nurse in Home Health to deliver high-quality nursing care in patients' homes, coordinate care with physicians, and ensure compliance with regulations. The role focuses on comprehensive care planning and collaboration with the health care team.

The RN will supervise home health aides, complete OASIS assessments, and participate in on-call rotations, with competitive pay and benefits within the Optum family.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current Driver's License and access to reliable transportation.
• Preferred CPR certification or ability to obtain within 90 days of hire.
• 1+ years of Home Health experience preferred.

Responsibilities

• Develop, implement, and revise the plan of care with the physician and health care team.
• Supervise care by home health aides and LPNs; assign tasks per regulations.
• Provide required supervisory visits and ensure regulatory compliance.
• Complete OASIS assessments and accurate medication reconciliation.
• Document patient visits per policy and payer requirements; coordinate with team.
• Communicate with physicians, patients, and families to ensure quality care.
• Participate in on-call and weekend rotations as needed.
