Job summary

You will follow physician care plans, administer medications and treatments, perform patient assessments, educate families, and coordinate with CNAs, HHAs, and case managers.

Qualifications

• Active RN license (State or Compact).
• 1+ year of RN experience (clinical or home health).
• CPR certification.
• TB test (or willingness to obtain).
• Reliable transportation.
• Liability insurance.

Responsibilities

• Follow physician-prescribed care plans.
• Administer medications and treatments.
• Conduct patient assessments and monitor vitals.
• Educate patients and families.
• Collaborate with CNAs, HHAs, and Case Managers.
• Document care accurately and in a timely manner.
• Perform various activities associated with daily living.
