Job summary

Addus Family Company is seeking a full-time Physical Therapist Assistant to provide compassionate in-home therapy services to elderly and disabled patients. Working under the direction of a Physical Therapist, you will implement treatment plans, assist with therapeutic exercises and mobility training, and monitor patient progress.

You’ll document care accurately, collaborate with an interdisciplinary team, and educate patients and families to promote safety and independence at home.

Qualifications

• Licensed Physical Therapist Assistant credential preferred; NM license or eligibility.
• Ability to deliver in-home PT services under supervision of PT.
• Strong documentation and communication with patients and families.

Responsibilities

• Implement treatment plans under PT supervision.
• Assist with therapeutic exercises and mobility training.
• Monitor patient progress and adjust activities as directed.
• Document care accurately and timely in patient records.
