Job summary

Fox Rehabilitation is seeking licensed Physical Therapist Assistants to provide in-home, one-on-one care to older adults in Carson City, NV. You’ll implement treatment plans with a supervising PT, guide patients through exercises, and document progress in FOX’s EMR within the Geriatric House Calls model delivered where patients live.

Qualifications

• Valid Physical Therapist Assistant license in the state(s) of practice, or eligibility to obtain.
• Graduate of an accredited Physical Therapist Assistant program
• Current CPR certification

Responsibilities

• Deliver individualized treatment plans established by your supervising Physical Therapist.
• Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities.
• Document visits accurately in FOX’s EMR system.
• Coordinate care with patients, POAs, and referral sources as needed.
