Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to provide one-on-one care in Carson City, NV, delivering geriatric-focused in-home and senior living care. The role emphasizes autonomy, flexible scheduling, and the ability to build long-term patient relationships.

Clinicians may use telehealth as appropriate, participate in a collaborative interdisciplinary team, and access ongoing professional development and reimbursement for credentials and CE opportunities.

Qualifications

• State-licensed Physical Therapist in NV or eligible
• Degree from an accredited Physical Therapy program
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in home or community settings
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to improve mobility and independence
• Document in FOX’s EMR system per regulatory standards
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
