FOX Rehabilitation is seeking licensed Physical Therapist Assistants in Boyertown, PA to deliver one-on-one in-home care under a supervising PT, helping older adults rebuild strength and mobility through the Geriatric House Calls model.

Qualifications

• Licensed Physical Therapist Assistant or eligible for PA licensure.
• Graduate of an accredited PTA program.
• Current CPR certification.
• Basic computer skills; MS Office proficiency.

Responsibilities

• Deliver individualized treatment plans by supervising PT and adjust activities based on patient progress.
• Instruct and guide patients through therapeutic exercises and functional activities.
• Document visits accurately in FOX's EMR following documentation standards.
• Coordinate with patients, POAs, and referral sources for coverage.
