Fox Rehabilitation is seeking motivated, licensed Physical Therapist Assistants for in-home and community-based care in Oceanside, NY. You will work closely with a supervising Physical Therapist to implement individualized treatment plans and guide patients through exercises to rebuild strength and confidence in their own homes and senior living communities.

Formación

• Licensed PTA required to practice in NY or eligible to obtain licensure.
• Graduate of an accredited Physical Therapist Assistant program.

Responsabilidades

• Deliver individualized treatment plans established by the supervising Physical Therapist.
• Instruct, motivate, and safeguard patients through therapeutic exercises and activities; track progress.
• Document visits accurately in FOX’s EMR system per Clinical Documentation standards.
• Coordinate with patients, POAs, and referral sources to ensure coverage when needed.
