Job summary

FOX Rehabilitation in Darien, CT is seeking a licensed Physical Therapist Assistant to work in-home with older adults under a supervising PT. You will deliver individualized treatment plans, guide patients through exercises, and help them regain strength, mobility, and confidence in their own homes.

Qualifications

• Valid PTA license in the applicable state(s) or eligible to obtain.
• Graduate of an accredited PTA program.
• Current CPR certification.
• Basic computer skills; proficient in Microsoft Office.

Responsibilities

• Deliver individualized treatment plans created by supervising PT and adjust activities based on progress.
• Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities.
• Document visits accurately in FOX’s EMR per clinical standards.
• Coordinate with patients, POAs, and community sources; help coverage when needed.
