Job summary

GrandCare Health Services seeks a Physical Therapist Assistant (PTA) to provide in-home therapy across the SFV area. Schedule: part time; you will work under a licensed Physical Therapist to deliver hands-on treatments and help patients regain mobility in a home setting.

You will educate patients, document progress, and coordinate care with strong clinical support. Requirements include California PTA license and CPR, with preferred home health experience.

Qualifications

• California PTA license required.
• Current CPR certification required.
• Minimum 1 year PTA experience; home health experience preferred.
• Reliable transportation and valid driver’s license.

Responsibilities

• Deliver hands-on physical therapy treatments designed to relieve pain, restore function, and improve mobility.
• Assist patients with therapeutic exercises, gait training, transfer activities, and prosthetic training.
• Apply physical agents including heat, cold, ultrasound, and massage.
• Educate patients on the proper use of assistive devices such as wheelchairs, braces, crutches, and other mobility equipment.
• Complete clinical documentation accurately and in a timely manner.
• Communicate patient progress, concerns, and changes in condition to the supervising Physical Therapist.
