Job summary

Summit Home Care & Hospice seeks an experienced Physical Therapist Assistant (PTA) to join our team providing in-home care across Dublin, OH and surrounding areas. The role involves direct patient therapy, collaboration with PTs, and travel to patient homes with flexible hours and on-call responsibilities.

Candidates must hold an Ohio PTA license, have an associate degree, and demonstrate strong communication, organization, and compassionate patient interaction.

Qualifications

• Associate Degree in Science from an accredited institution.
• Active PTA license in Ohio.
• Willingness to travel to patients' homes to provide care.
• Excellent verbal and written communication and organizational skills.
• Ability to build rapport with patients, PT staff, ALF/ILF teams.

Responsibilities

• Work directly with patients on therapeutic exercises to regain mobility.
• Administer modalities to relieve pain and improve circulation.
• Collaborate with team to deliver comprehensive care.
• Develop and implement PT programs for rehabilitation.
• Evaluate progress and adjust plans as needed.
• Record and document patient care and progress.
• Provide training for using assistive devices.
