Job summary

Summit Home Care and Hospice is seeking an experienced Physical Therapist Assistant (PTA) to support patients in Dublin, OH. You will travel to homes to guide therapeutic exercises and administer modalities, contributing to mobility and pain relief while coordinating with PTs and staff.

The role requires an Ohio PTA license, an Associate Degree, and a willingness to travel to patient homes. Strong communication and organizational skills are essential for success in a fast-paced home-care setting.

Qualifications

• Associate degree required from an accredited institution.
• Active PTA license in Ohio.
• Willingness to travel to patients' homes for care.
• Previous experience in physical therapy, rehabilitation preferred.
• Excellent verbal and written communication and organizational skills.

Responsibilities

• Work directly with patients on therapeutic exercises to regain mobility and strength.
• Administer modalities (e.g., soft tissue massage, electrical stimulation, ultrasound).
• Collaborate with team to achieve comprehensive patient care.
• Develop and implement PT programs focused on rehabilitation.
• Evaluate progress and adjust plans of care as needed.
• Record and document patient care services and progress.
