Job summary
Always Best Care Senior Services on Oʻahu seeks a licensed Physical Therapist to join our home health team. Provide skilled PT services in patients' homes, with focused evaluations and personalized treatment plans.
This full-time position offers flexible scheduling, per-visit compensation, SOC visits, and opportunities to build your own caseload while helping seniors stay safe and independent at home.
Qualifications
- Current Hawaiʻi Physical Therapist (PT) license in good standing.
- Minimum of one year of clinical experience preferred.
- Home Health experience is a plus, but not required.
- Strong clinical judgment, communication, and documentation skills.
- Valid driver's license, reliable vehicle, and current auto insurance.
- Ability to travel throughout Oʻahu to provide in-home patient care.
- Passion for serving seniors with compassion and professionalism.
Responsibilities
- Provide skilled physical therapy services in patients' homes.
- Perform comprehensive initial evaluations, reassessments, and fall risk assessments.
- Develop individualized treatment plans to improve strength, mobility, balance, and functional independence.
- Educate patients and caregivers on home exercise programs and injury prevention.
- Collaborate with nurses, physicians, and the interdisciplinary care team to ensure quality patient outcomes.
- Complete accurate, timely, and compliant clinical documentation.
