Job summary

O'ahu Home Health is hiring licensed Physical Therapists (PT) and Physical Therapist Assistants (PTA) for per diem home visits on O'ahu, HI. This flexible schedule lets you choose your own patients while delivering in-home care to seniors.

Responsibilities include in-home visits, evaluations, reassessments and fall risk screening, plus accurate documentation and collaboration with the care team. A Hawaii PT or PTA license, 1+ year of clinical experience, and a valid driver’s license are required.

Qualifications

• Current Hawaii license as PT or PTA.
• 1+ year of clinical experience (home health preferred).
• Strong documentation and communication skills.
• Valid driver’s license and reliable transportation.

Responsibilities
