Job summary

O'ahu Home Health Agency is hiring licensed Physical Therapists (PT) and Physical Therapist Assistants (PTA) for per diem, in-home visits across O'ahu, HI.

You will perform evaluations, follow fall-prevention and mobility goals, document progress, and communicate with the care team; a Hawaii license and reliable transportation are required.

Flexible scheduling lets you choose patients while helping seniors stay independent at home.

Qualifications

• Current Hawaii license as PT or PTA.
• 1+ year of clinical experience (home health preferred).
• Strong documentation and communication skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide in-home physical therapy visits.
• Complete evaluations, reassessments, and fall risk assessments.
• Document accurately and communicate progress with the care team.
