Job summary

TalentBurst is seeking a licensed Physical Therapist for an in-home outpatient rehabilitation role in Skowhegan, ME. The position involves evaluating and treating older adults in home or community settings, developing individualized plans, and coordinating with the rehab team.

The role requires a PT license and advanced degree in PT. The assignment runs 15 weeks with a schedule of 8 hours per day, Monday through Friday, onsite.

Qualifications

• Must hold a valid Physical Therapy license in the applicable state.
• Master’s degree in Physical Therapy; or Master’s in a PT-related field with a PT degree.
• Bachelor’s degree in Physical Therapy plus 5 years of PT experience.

Responsibilities

• Provide in-home outpatient rehabilitation services to older adults.
• Develop and implement individualized PT plans of care.
• Coordinate referrals to other disciplines as needed.
• Supervise PT Assistants and students as allowed by licensure.
• Document clinical notes and treatment plans in a timely manner.
