Job summary

InHome Therapy is seeking licensed physical therapists to deliver in-home PT services to adults and geriatric patients in and around St. Charles, IL.

You will evaluate mobility, design individualized treatment plans, and implement therapeutic exercises in patients' homes with a focus on functional improvement and safe discharge. The role offers a flexible schedule with autonomy, a consistent caseload through our agency network, mileage reimbursement, and paid training.

Qualifications

• Active physical therapy license in good standing
• BLS CPR certification
• Reliable transportation to home visits
• Comfort with technology (email, texting, EMR) with training provided

Responsibilities

• Deliver in-home physical therapy services to adult and geriatric patients
• Evaluate mobility, strength, and function; develop individualized goals
• Implement rehabilitation and therapeutic exercise programs
• Educate patients and families on recovery plans and preventive care
• Maintain accurate documentation per Medicare and agency standards
