Job summary

InHome Therapy is seeking a licensed Physical Therapist to deliver in-home care to adults and seniors in the Hoffman Estates area. You will evaluate mobility, design individualized rehab plans, and guide patients through exercises in their own homes.

You’ll work with a supportive team, enjoy a flexible schedule, mileage reimbursement, and per-visit compensation ranging from $65 to $85. PTO and paid holidays are provided where eligible.

Qualifications

• Active physical therapy license in good standing.
• BLS CPR certification.
• Reliable transportation to see patients in their homes.
• Comfort with technology and EMR systems (training provided).

Responsibilities

• Deliver in-home physical therapy services to adult and geriatric patients in your community.
• Evaluate patient mobility, strength, and function to develop individualized goals.
• Implement effective rehabilitation and therapeutic exercise programs.
• Educate patients and families on recovery plans and preventive care.
• Maintain accurate home health documentation per Medicare and agency standards.
