Job summary

TheraCare is seeking a licensed Physical Therapist (PT) for a full-time home health role serving adult and geriatric patients in Beaumont, TX. You will provide in-home PT services, create one-on-one care plans, and help patients improve mobility, independence, and safety.

Qualifications

• Doctorate in Physical Therapy from an APTA-accredited program.
• Active PT license in Texas or eligible for Compact Licensure.
• Home health experience (1 year required).
• HomeCareHomeBase (HCHB) experience preferred.
• Current CPR certification.
• Knowledge of OASIS and home health regulations preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform home health PT evaluations and re-evaluations, including OASIS assessments, for adults and geriatrics.
• Develop and update individualized plans of care based on patient goals and physician orders.
• Provide skilled PT treatment for orthopedic, neurological, cardiac, post-surgical, and medically complex conditions.
• Deliver evidence-based interventions focusing on mobility, gait, balance, strengthening, fall prevention, pain management, and functional independence.
• Educate patients and caregivers on home exercise programs, safety, transfers, fall prevention, and equipment use.
• Monitor patient progress and modify treatment plans as needed.
• Complete accurate documentation within the EMR at the point of care.
• Collaborate with physicians, nurses, case managers, PTAs, aides, and interdisciplinary teams.
• Supervise PTAs and home health aides per state regulations and agency policies.
• Participate in case conferences, discharge planning, and care coordination.
• Ensure compliance with home health regulations and infection control standards.
• Maintain professional competency through ongoing education and training.
