Qualifications

• Current unrestricted CA Physical Therapist license required.
• BLS/CPR certification necessary.
• Reliable vehicle and valid CA driver’s license needed.
• Experience in home health and high‑acuity care is preferred.
• Knowledge of California home‑care regulations and community resources is essential.
• Experience with interdisciplinary teams and case management is highly desired.

Responsibilities

• Perform in‑home evaluations and develop individualized care plans.
• Coordinate with PTAs to ensure plan adherence and support patient independence.
• Assist patients with mobility and daily activities.
• Communicate progress to physicians and report to MDs.
• Document all visits and maintain electronic charting.
