Job summary

Qualifications

• Must have valid CA Driver's License.
• Knowledge and understanding of individual development and behavior related to illness/injury.
• Experience in home health care is preferred.

Responsibilities

• Perform an evaluation in the home and develop a care plan with PTAs.
• Help patients regain their independence.
• Communicate and coordinate care with patients’ physicians.
