Job summary

Senior Care Solutions Group is seeking a compassionate Field Psychiatric RN to provide in-home nursing services on a per diem basis in Hallandale Beach, FL. You will assess patients, develop care plans with the medical team, and deliver education to patients and families, promoting stabilization and independence.

Ideal candidates hold an active Florida RN license with 1+ year of psychiatric or mental health home health experience, and can work independently with a flexible schedule.

Qualifications

• Active Florida RN license.
• 1+ year of psychiatric/mental health nursing (home health).
• Prior home health experience.
• Strong assessment, clinical judgment, and communication skills.
• Reliable transportation and valid driver's license.
• Ability to work independently with a flexible per diem schedule.

Responsibilities

• Conduct in-home psychiatric nursing assessments and evaluations.
• Develop and implement individualized care plans with physicians and the interdisciplinary team.
• Provide skilled psychiatric nursing care and education to patients and caregivers.
• Monitor and document changes in patient condition, behavior, and medication response.
• Coordinate care with other clinicians, therapists, and support services.
• Ensure timely and accurate clinical documentation in accordance with agency policies and regulatory guidelines.
• Educate patients and families about mental health conditions, medication compliance, and coping strategies.
• Respond to crises or urgent patient needs as appropriate within scope of practice.
• Maintain compliance with Medicare, state, and agency standards.
