Qualifications

• Graduated from an accredited school of professional nursing.
• Current BLS Certificate.
• Valid Driver's License.

Responsibilities

• Demonstrate efficient teamwork with the staff.
• Perform nursing procedures according to agency policy.
• Work under the direction of a RN Case Manager and Director of Nursing.
• Monitor patient progress using observation, assessment, and evaluation skills.
• Educate patients and family on disease process, medications, and home care procedures.
• Document visits and complete daily case notes; notify Case Manager of adverse findings.
• Follow HIPAA and infection control policies.
• Transport patients to appointments, due, and pharmacies.
