Job summary

Avid Health at Home is seeking a Private Duty Nurse (PDN)-LPN to join their in-home care team. The LPN will provide skilled nursing services in clients' residences under the supervision of the Clinical Manager.

Responsibilities include evaluating clients, supporting care plans, documenting care, and maintaining safety and infection control. In-person work location with opportunities for part-time and PRN shifts.

Qualifications

• Graduate of an accredited school of nursing.
• Licensure as LPN in the applicable state with current status.
• Travel is required for business purposes.
• Minimum of one year home care experience; extra year for training classes.
• Full time is at least 30 hours per week (Sat–Fri).
• Bi-lingual English/Spanish a plus.

Responsibilities

• Evaluates and treats clients under supervision of an RN/CM.
• Assist with continuity of care and emotional needs of client/family.
• Document communications and client care per agency policies.
• Ensure infection control and safety practices are followed.
• Maintain accurate clinical and shift documentation.
