Job summary

Interim HealthCare Inc. seeks a Private Duty Nurse (PDN) LPN to deliver 1:1 in-home skilled nursing care, supporting longer shifts and building trusted relationships with clients and families in Ohio. The role emphasizes comfort, safety, and dignity, enabling clients to stay at home.

You will provide consistent 1:1 care, follow care plans, and collaborate with the care team to achieve goals. CPR/BLS certification and reliable transportation are required.

Qualifications

• Graduate of an accredited practical nursing program and current LPN/LVN license in the practice state.
• One year of home care/private duty experience preferred or completion of the office's preceptor program.
• Current CPR/AED/BLS/First Aid certification.

Responsibilities

• Provide 1:1 in-home skilled nursing care per the plan of care.
• Coordinate with client, family/caregivers, and care team to ensure continuity.
• Assess, monitor, and document patient condition; report changes promptly.
