Job summary

Interim HealthCare Inc. is seeking a Private Duty Nurse (PDN) LPN for 1:1 continuous in-home care. You will deliver skilled nursing in clients' homes, building lasting relationships with clients and families while maintaining safety and comfort.

Responsibilities include administering medications, documenting care, and coordinating with the care team to ensure seamless coverage. A valid LPN/LVN license and CPR certification are required.

Qualifications

• Graduate of an accredited practical (vocational) nursing program.
• Current LPN/LVN license in the practice state(s).
• CPR/AED/BLS/First Aid certification.
• Reliable transportation to care sites or work locations.
• Trach/ventilator experience preferred; training may be available.

Responsibilities

• Provide 1:1 in-home skilled nursing care per care plan.
• Coordinate with client, family, and care team for goals of care.
• Assess, monitor, document patient condition; report changes.
• Administer medications and treatments with shift-based notes.
• Perform wound care and personal care as outlined in the plan.
• Identify signs of infection or complications and respond per protocol.
• Educate client and family on routines and equipment use.
• Complete timely clinical notes and shift handoff information.
• Communicate with supervising RN and office team to ensure coverage.
