Job summary

Signify Health, part of CVS Health, is seeking a Part Time Physician to perform In-Home Health Evaluations for members in their own homes. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests.

Clinicians travel locally within licensed states, may conduct virtual visits, and are not required to prescribe or alter treatment.

Qualifications

• Active, unrestricted license(s) in coverage area(s).

Responsibilities

• Provide in-home health evaluations during 45-minute visits.
• Travel locally within licensed states as needed; conduct virtual visits when required.
• Engage with members and families to bridge gaps in care; document evaluations in workflow.
