Job summary

Signify Health is seeking a Per Diem Physician to perform in-home health evaluations, connecting patients with the right care at their own doors. You will assess medical history, perform physicals, and coordinate with health plans, all while using an iPad-based workflow.

The role is part-time with a 12-hour weekly expectation and travel within licensed states. Active MD or DO licenses are required, with multi-state licensure preferred.

Qualifications

• Active, unrestricted license in the coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Provide in-home health evaluations to plan members.
• Review medications and medical history during visits.
• Perform physical evaluations and follow the clinical workflow on an iPad.
• Travel locally within licensed states as needed; conduct virtual visits when required.
• Offer diagnostic and preventive services as indicated (e.g., spirometry, etc.).
