Job summary

CVS Health / Signify Health is seeking a Part Time In-Home Physician to deliver health evaluations in members' homes in the Toledo area. You will conduct 45-minute visits, review medications and medical history, perform a physical evaluation, and coordinate with health plans for additional tests as needed.

The role requires an active MD or DO license, ability to travel within licensed states, and a commitment to high-quality, patient-centered care.

Qualifications

• Active, unrestricted medical license in the coverage area.
• Multi-state licenses preferred.
• Board-certified or eligible in family medicine or internal medicine.

Responsibilities

• Provide in-home health evaluations for CVS/Signify Health members.
• Engage with patients in their homes and perform medical assessments.
• Travel locally to perform visits; virtual visits as needed.
