Job summary

Signify Health, part of CVS Health®, seeks a Part Time Physician to deliver in-home health evaluations, engaging with people in the comfort of their homes and bridging gaps in care. You’ll visit members nationwide, perform evaluations, and use an iPad with a clinical workflow while not prescribing or altering current treatments.

Expect a 29.5-hour weekly schedule with a flexible mix of in-home and virtual visits, and a comprehensive benefits package for part-time staff.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Visit patients in their homes for a 45-minute health evaluation and build trusted relationships.
• Bridge gaps in care by connecting patients to the right health services and plan recommendations.
• Travel locally to visit members in licensed states and conduct virtual visits as needed.
• Provide diagnostic and preventive services as indicated by the health plan.
