Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to administer PT treatment plans in patients' homes, under the supervision of a Physical Therapist. You will perform non-complex exercises, massages, and modalities while documenting care and coordinating with the team.

The role requires a valid physical therapy license, CPR certification, and a current driver's license. English fluency and reliable transportation are essential for this full-time position in a home

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric physical, emotional, social changes.
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in their homes.
• Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, electrical modalities.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, supply issues, or staff or client changes.
• Apply infection control and universal precautions in patient care.
