Job summary

Humana's CenterWell Home Health seeks a licensed Physical Therapist Assistant to support patients in their homes, implementing the plan developed by the Physical Therapist and focusing on functional improvement. You will administer non-complex manual therapies, document patient records, attend educational meetings, and coordinate with the care team, while following infection control protocols.

The role requires a valid state PT license, CPR, a driver's license, and English proficiency; the pay

Qualifications

• Current state license to practice Physical Therapy
• Valid driver's license and reliable transportation
• Current CPR certification
• Experience collaborating with a health-care team
• Knowledge of geriatric physical, emotional, social and biological changes
• Fluent English (read, write, speak)

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes as developed by the Physical Therapist
• Administer non-complex manual therapies, therapeutic massage, traction, heat, light, cold, water and electrical modalities
• Document client records and submit clinical documentation weekly
• Attend educational meetings and coordinate department in-services
• Notify PT or Branch Director of equipment failures, supply shortages or staff changes
• Apply infection control and universal precautions in patient care
