Job summary

CenterWell Home Health is seeking a Home Health Physical Therapy Assistant to help patients regain strength and independence in their homes, reporting to the Clinical Manager. You’ll deliver hands-on PT care as part of a supportive, patient-centered team.

Responsibilities include administering the treatment plan created by a Physical Therapist, documenting progress, coordinating with staff, and ensuring infection control.

Qualifications

• Team collaboration and communication with a care team.
• Fluent English for patient interaction and documentation.
• CPR certification required to perform in-home care.

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes as developed by the Physical Therapist.
• Administer non-complex manual therapeutic exercises, massage, traction, heat, cold, and electrical modalities as appropriate.
• Document client records and submit clinical documentation on time each week.
