Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to join our caring community. You will support patients in their homes under the supervision of a Physical Therapist to help regain strength, mobility and independence while delivering personalized care focused on functional improvement.

Responsibilities include executing the prescribed treatment plan, documenting progress weekly, and coordinating with the team.

Qualifications

• Current state license to practice Physical Therapy is required.
• Valid driver’s license, auto insurance and reliable transportation are required.
• Current CPR certification is required.
• Experience collaborating with a team of healthcare professionals is required.
• Comprehensive knowledge of geriatric patient changes (physical, emotional, social, biological).
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in patients' homes.
• Administer non-complex manual therapy exercises, massage, traction heat, light, cold, water, and electrical modalities.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, supply issues, or client changes.
• Apply infection control and universal precautions in patient care activities.
