Job summary

PeaceHealth Sacred Hearts Home Health in Springfield, OR seeks a Physical Therapy Assistant (PTA) for full-time, 1.0 FTE, day position. Salary range is $31.60–$47.40 per hour; hiring rate based on education, experience and collective bargaining terms.

Work in patients’ homes, coordinating with a multidisciplinary team and caregivers to restore function and independence. Responsibilities include providing skilled PT services under a PT supervision, documenting per standards, and mentoring therapy

Qualifications

• Associate Degree required: PTA accredited by CAPTE.
• One year inpatient experience; home health/geriatric preferred.
• Current Basic Life Support and Oregon licensure; valid driver's license with auto insurance.

Responsibilities

• Provide skilled physical therapy services under a licensed PT.
• Complete documentation accurately and timely per regulatory standards.
• Collaborate with healthcare team to ensure continuous patient care.
