Job summary

PeaceHealth Sacred Hearts Home Health in Springfield, OR, is hiring a Physical Therapy Assistant - Home Health for a full-time, 1.00 FTE, day shift. Salary range is $31.60–$47.40 hourly; hiring rate depends on education, experience, and bargaining terms.

You’ll work in patients’ homes with a multidisciplinary team, improving functional outcomes. You will collaborate with therapists, nurses, social workers, and aides, while mentoring PTA students and contributing to program development.

Qualifications

• Associate degree as Physical Therapy Assistant accredited by CAPTE.
• At least 1 year experience in inpatient setting; home health/geriatric preferred.
• Licensure as PTA in Oregon; temporary permit allowed up to 3 months; Basic Life Support.
• Driver's license and vehicle insurance proof.
• Excellent verbal and written communication, strong organizational and analytical skills.

Responsibilities

• Provide skilled physical therapy services under the supervision of a licensed PT.
• Complete documentation accurately and timely per regulations and standards.
• Collaborate with healthcare professionals to ensure continuous care.
• Mentor therapy aides and supervise PTA students.
• Participate in program development, quality improvement, and staff meetings.
