You’ll work in patients’ homes with a multidisciplinary team, improving functional outcomes. You will collaborate with therapists, nurses, social workers, and aides, while mentoring PTA students and contributing to program development.

Qualifications

  • Associate degree as Physical Therapy Assistant accredited by CAPTE.
  • At least 1 year experience in inpatient setting; home health/geriatric preferred.
  • Licensure as PTA in Oregon; temporary permit allowed up to 3 months; Basic Life Support.
  • Driver's license and vehicle insurance proof.
  • Excellent verbal and written communication, strong organizational and analytical skills.

Responsibilities

  • Provide skilled physical therapy services under the supervision of a licensed PT.
  • Complete documentation accurately and timely per regulations and standards.
  • Collaborate with healthcare professionals to ensure continuous care.
  • Mentor therapy aides and supervise PTA students.

PeaceHealth Sacred Hearts Home Health team in Springfield, OR is seeking a Physical Therapy Assistant - Home Health for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $31.60 – $47.40. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.


You will have the opportunity to work in the patient’s own environment engaging family and caregivers.


Our multi-disciplinary Home Health team consists of Physical Therapists, Occupational Therapists, Speech Language Pathologists, RN’s, Medical Social Workers, Registered Dietitians and Bath Aides. Our dynamic team meets regularly providing the opportunity to collaborate with supportive peers to coordinate care and achieve optimal outcomes.

Job Summary

Responsible for carrying out the physical therapy plan of care, toward patient-specific goals, under the direct supervision of a licensed physical therapist. Provides treatment to restore function, prevent disability and assist patients to achieve optimal levels of functional status. Provides oversight to students in following the plan-of-care and treatment of patients appropriate to the age of the patient served; provides supervision to therapy aides and volunteers. Works as part of a patient care team toward the best outcomes for the patient.


What you will do:


  • Under the supervision of the physical therapist, provides skilled physical therapy services to patients and skilled education/instruction to patients and caregivers according to the plan of treatment.

  • Completes documentation accurately, timely, according to regulatory and state licensure requirements, and within professional standards.

  • Collaborates with other healthcare professionals as necessary to ensure smooth continuous care of the patients. May act as a liaison with nursing, medical and other therapy staffs to facilitate problem solving and coordination of other services and act as an educational resource.

  • Mentors and provides oversight to therapy aides. Provides clinical instruction and training to physical therapy assistant students.

  • Recommends and participates in development of new services and programs, quality improvement and monitoring activities.

  • Participates in staff meetings, patient care meetings, educational in-services, caregiver orientation and other professional activities.


Qualifications

Education


  • Associate Degree Required: Physical Therapy Assistant accredited by the Commission on Accreditation in Physical Therapy Education


Experience


  • Required one-year experience in an inpatient setting, with home health/geriatric experience preferred.


Credentials


  • Required: Upon Hire Basic Life Support and

  • Required: Licensed Physical Therapist Assistant - Oregon For positions in Oregon, a temporary permit is acceptable under the rules set forth by the Oregon State Board of Physical Therapy. A temporary permit shall be allowed for a period not to exceed three months or

  • Required: Drivers License and proof of auto insurance

Working Conditions

Lifting



  • Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH).

  • Fine motor skills to be able to grasp and control medical equipment and perform precise procedures.

  • Ability to move around area with occasional sitting.

  • Lifting (non-patient)/carrying occasionally up to 25 lbs.

  • Push/Pull: 1-25 lbs. of force occasionally (i.e., moving equipment).

  • Stooping/ bending/ squatting/ kneeling occasionally.

  • Rehabilitaion staff working with patients with rehabilitation goals involving bed mobility, transfers and gait may use other appropriate and approved safe techniques and equipment in the course of the rehabilitation process if determined clinically appropriate.


Environmental Conditions


  • Exposure to biohazard, body fluids and airborne particles.

  • Time spent on site in medical/hospital setting.


Mental/Visual


  • Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).

  • Ability to communicate and exchange accurate information.
