Job summary

National Church Residences seeks a Physical Therapist Assistant for our Central Ohio Home Health program serving Columbus, Dublin, Westerville, and surrounding areas. The PTA will work Monday–Friday under a supervising PT to implement treatment plans, document progress, and educate patients and families in their homes.

The role requires an Associate Degree in Physical Therapy, Ohio licensure, and 2–4 years of PTA experience if possible.

Qualifications

• Associate Degree in Physical Therapy required.
• Two to four years of PTA experience preferred.
• Licensed in the state of practice required.

Responsibilities

• Provide and direct therapy rehabilitation services under supervision.
• Develop and evaluate plan of care with patient and caregiver.
• Educate patients and caregivers on treatment plans and progress.
• Maintain accurate documentation per facility policy.
• Communicate patient progress to supervising PT.
