Job summary

PeaceHealth Sacred Hearts Home Health in Springfield, OR is seeking a Physical Therapy Assistant - Home Health for a Full Time, 1.00 FTE, Day position. The role delivers skilled PT services in the patient’s home environment, under the supervision of a licensed PT, and involves collaboration with a multidisciplinary team.

Ideal candidates have an Associate Degree in Physical Therapy and at least one year of inpatient experience, with home health/geriatric experience preferred.

Qualifications

• Associate degree required in Physical Therapy Assistant accredited by CAPTE or equivalent.
• One year inpatient experience; home health/geriatric experience preferred.
• Excellent verbal and written communication skills; ability to work with patients and caregivers.

Responsibilities

• Provide skilled physical therapy services to patients under the supervision of a licensed PT.
• Complete documentation accurately and timely per regulatory standards.
• Collaborate with healthcare professionals to ensure continuity of care.
• Mentor therapy aides and supervise PTA students.
• Contribute to development of new services and quality improvement initiatives.
• Participate in staff meetings and caregiver education sessions.
