Qualifications

• Active PT license in the state of practice
• CPR certification on file
• Experience collaborating with healthcare professionals
• Knowledge of geriatric care and mobility rehabilitation
• Read, write, and speak fluent English

Responsibilities

• Deliver the physical therapy treatment plan to patients, working onsite in patients' homes as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapies, therapeutic massage, traction, heat, light, cold, water and electrical modalities as directed.
• Document client records and submit clinical documentation to the office weekly.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, supply issues, or staff or client changes.
• Apply infection control and universal precautions in patient care activities.
