Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Knowledge of geriatric physical, emotional and social changes.
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
• Perform non-complex manual therapies and modalities incl. therapeutic massage, traction, heat/cold, and electrical modalities.
• Document client records and submit clinical documentation on time weekly.
• Attend educational meetings and coord in-services for the department.
• Notify PT or Branch Director of equipment failures, supply issues or staff changes.
• Apply infection control and universal precautions in patient care.
