Job summary

The Employee Connect is seeking a licensed Physical Therapist to join our Home Health team in Sterling Heights, Michigan. You will evaluate patients, develop individualized treatment plans, and deliver skilled PT services in patients' homes, fostering autonomy and strong patient relationships.

Qualified candidates hold a Michigan PT license, a Doctor of Physical Therapy degree, and at least 1 year of clinical experience (home health preferred).

Qualifications

• Licensed Michigan Physical Therapist in good standing.
• Doctor of Physical Therapy degree from an accredited program.
• Minimum of 1 year of clinical experience (home health preferred).
• Valid driver's license and reliable transportation.
• Strong knowledge of Medicare home health regulations and documentation standards.
• Proficiency with electronic medical records systems.
• Excellent interpersonal and communication skills with a patient-first mindset.

Responsibilities

• Conduct initial evaluations and develop individualized treatment plans for patients.
• Deliver skilled physical therapy services in patients' homes within Sterling Heights and surrounding areas.
• Set measurable goals with patients and families.
• Document progress and ensure compliance with regulatory requirements.
• Coordinate care with referring physicians and the broader team.
• Educate patients and caregivers on home exercise programs and safety.
• Manage caseload efficiently while maintaining high clinical standards and documentation.
• Participate in case conferences and quality improvement activities.
