Qualifikationen

• Knowledge of therapy best practices and clinical judgment.
• Detail-oriented and observant.
• Valid driver’s license and auto insurance in your name.

Aufgaben

• Provide skilled physical therapy in patients' homes as ordered by the physician.
• Observe and report patient responses and progress to physicians and agency staff.
• Perform OASIS assessments to develop and adjust plans of care.
• Develop and implement care plans to restore function and relieve pain.
• Educate patients and families on proper use of equipment and home safety.
• Maintain documentation per agency protocols and Medicare guidelines.
• Coordinate with PTAs and home health aides on supervision and care delivery.
